    W-9 Tax Form

    Required so we can process your payments.

    Why we ask for this

    The IRS requires anyone who pays you for services to have your correct Taxpayer Identification Number (TIN) on file. Form W-9 is how independent contractors provide it.

    Roles at Premier Assist are classified by the IRS as either W-2 employment or 1099 independent contracting based on the nature of the work, not by choice. Your role is classified as 1099, which is why you're being asked to complete a W-9. Roles classified as W-2 employment use Form W-4 instead. Either way, every U.S. worker has some version of this paperwork. If you have questions about how your role is classified, ask your hiring manager before continuing.

    + Add a business name / DBA A DBA ("doing business as") name is the public name for your business if it's different from your legal name. Skip this unless you receive payments under a separate business name.

    W-4 Tax Form

    Required so we know how much federal tax to withhold from each paycheck.

    Why we ask for this

    The IRS requires every W-2 employee to file a Form W-4 with their employer. It tells Premier Assist how much federal income tax to withhold from each paycheck.

    Your role at Premier Assist is classified as W-2 employment, which is why you're being asked to complete a W-4. If you have questions about how your role is classified, ask your hiring manager before continuing.

    A few more questions about your situation

    Most people answer No to most of these - it's quick. We use your answers to figure out the right amount of federal tax to withhold from each paycheck.

    Do you have any children under 17 that you claim as dependents? A qualifying child is under 17 at year end, lives with you more than half the year, has a valid Social Security number, and is claimed as your dependent on your tax return. Each one gives you a $2,200 Child Tax Credit allowance on your 2026 W-4 (up from $2,000 in 2025).

    Do you support any other dependents (older children, parents, others living with you)? Other dependents are people you support and claim on your tax return who aren't qualifying children under 17. That includes adult children, parents, in-laws, or other relatives who depend on you. Each one gives you a $500 Credit for Other Dependents.

    Do you have income from any other source, like investments, a side business, or rental property? Interest, dividends, rental income, freelance or self-employment income, and similar non-job earnings don't have tax withheld automatically. Telling us the annual total lets us increase the withholding from your paycheck so you don't owe at tax time.

    Do you (or your spouse, if filing jointly) have a second job? When you (or your spouse, if filing jointly) work more than one job, the combined income may sit in a higher tax bracket than either employer is withholding for individually. Answering Yes tells us to add extra withholding so you don't get hit with a bill at tax time.

    Do both jobs pay roughly the same amount?

    Tell us roughly what each job pays in a year - we'll figure out the extra federal tax to withhold per paycheck so you're not underwithheld at tax time.

    Did you earn tips, work paid overtime, or pay interest on a vehicle loan this year? 2026 W-4 line 1: above-the-line deductions for qualified tips (up to $25,000), qualified overtime compensation (up to $12,500 / $25,000 MFJ of the "and-a-half" portion), and qualified passenger-vehicle loan interest (up to $10,000). All three phase out for high earners.

    Enter what applies. Income limits kick in around $150,000 single / $300,000 MFJ for tips and overtime, and $100,000 / $200,000 for vehicle loans - we apply them automatically.

    Any student loan interest, IRA contributions, or other above-the-line adjustments? 2026 W-4 line 5: covers Schedule 1 (Form 1040) Part II adjustments to income - student loan interest, deductible IRA contributions, alimony paid (for older agreements), educator expenses, HSA contributions, and a few other items. Most people leave this blank.

    Do you plan to itemize deductions on your tax return? Most people don't (they take the standard deduction). For 2026 the standard deduction is $16,100 if you file single or MFS, $32,200 if MFJ or qualifying surviving spouse, and $24,150 if head of household. Itemize only if your total mortgage interest, state and local taxes (SALT capped at $40,400 / $20,200 MFS), charity, and large medical bills add up to more than that.

    Estimate each category for the year. We apply the 2026 SALT cap ($40,400), the 7.5% income floor for medical, and the 0.5% income floor for charity automatically - then compare against the standard deduction and use whichever is larger.

    Only the portion above 0.5% of your total income counts toward itemizing (new for 2026). Up to $1,000 / $2,000 MFJ of cash gifts can also be claimed if you take the standard deduction.

    Why we ask for this

    New York Labor Law § 195(1) requires every employer to give every new W-2 employee a written notice of their pay terms - rate, basis, overtime rate, regular payday, and employer information - on or before their first day of work. The terms below are what your hiring manager set up for you. Confirm they're correct, then sign to acknowledge receipt.
